Procedure for Questionnaire Creation
Accessing the Admin Area:
Start by clicking the gear icon located in the upper right corner of the Supplier Management page.
Switching to Questionnaire Management:
Select "Supplier Management" on the left, then choose "Supplier Questionnaires" below it to manage your questionnaires.
Creating a New Questionnaire:
Click "New Questionnaire" in the upper right corner to begin crafting a new questionnaire.
Adding Title and Description:
Assign your questionnaire a meaningful title and provide a clear description. Then, click "Update" to save your title and description.
Example:
Title: Quality Management
Description: Please answer the following questions regarding your Quality Management system.
Activating Edit Mode:
Use the pencil icon to edit and/or design your questionnaire.
Organizing Sections:
Questionnaires are divided into one or more sections for clear structure and thematic segmentation. Customize your questionnaires to meet your specific requirements. If a topic is completely different, consider creating a new questionnaire to maintain clarity and thematic relevance.
Adding Sections:
Create a new section by clicking "Add Section" and giving it a relevant title and description.
Integrating Question Types:
Enhance the questionnaire with different types of questions that appear on the right side. These options enable versatile data collection.
Descriptions of Listed Field Types:

Link your questions intelligently so they can adapt based on participants' previous responses.
Creating Conditional Logic:
- Choosing the Question: Identify the question in your questionnaire that you want to apply conditional logic to, referred to as the "conditional question".
- Creating Conditional Logic: Click on the "Create conditional question" button for the selected question to set up conditions.
- Defining the Condition: Precisely specify the circumstances under which the conditional question should be displayed. This is done by selecting another question (the "condition question") and the specific answer(s) to that question that will trigger the display of the conditional question.
- Accuracy and Evaluation: Ensure the condition is precisely defined to keep the logic of your questionnaire accurate and coherent. The condition should be clear and direct for a correct evaluation of participant responses.
- Activating the Condition: Save the defined condition to activate it. Only after saving will the conditional logic take effect and be evaluated during the response process.
Scoring with Points:
Use points instead of fixed percentages to define the importance of a question. Each point contributes to the weighting relative to other questions.
Automatic Response Scoring:
Determine whether responses should be automatically scored.
Don't forget to save the settings for each question by clicking the "Save" button.
Scoring Logic for Multiple Choices:
For questions allowing multiple responses, consider how points will be allocated. You can assign points to each set of answers or accumulate points based on the selected responses.
Final Score:
The final result of the questionnaire is derived from all the points of the answered questions.
Reviewing the Final Score:
Carefully review the final score of the questionnaire.
Finalizing the Questionnaire:
Go through the entire questionnaire and save it to activate the automatic scoring function.
